Abstract |
Background
General Practitioners’ adherence to practice guidelines is variable. Barriers to guideline implementation have been identified and extensively studied but the results of qualitative research remain spread. Synthesis and in depth analysis of the published results is necessary to interpret the causes of this phenomenon.
Aim
To identify and synthesize qualitative research on barriers to adherence of clinical practice guidelines by Family Physicians and their implementation in Primary Health Care.
Design of the Study
Systematic Review, meta synthesis of qualitative studies.
Methods
Electronic searches have been conducted in selected bibliographic databases (MEDLINE, EMBASE, HMIC, WEB OF SCIENCE) which were used as data sources. Identified studies were screened for inclusion according to selection criteria set by the reviewer. Critical appraisal of the methodological quality of the studies identified was then followed by data extraction carried out by the reviewer. The data were classified in descriptive themes. Finally analysis and meta- synthesis of the data collected lead to the final conclusions.
Results
There is a variety of barriers to guidelines adherence not primarily related to socio-demographic factors of General Practitioners. Lack of skill and knowledge on guidelines, scepticism of their feasibility and patient’s special characteristics were the most important causes for non-compliance with the recommendations. Lack of time
and resources within the healthcare system can prevent implementation of clinical guidelines in daily practice of Family Medicine.
Conclusion
Compliance with clinical guidelines is directly affected by Physicians’ level of knowledge and their relationship with patients.
The external environment can influence the process of clinical decision making in an extent degree. Planning and implementation of integrated policies for clinical governance and clinical audit is needed taking into account the complex nature of General Practitioner’s task and needs within Primary Healthcare.
