Abstract ATRIAL FIBRILLATION Atrial fibrillation is the most common sustained arrhythmia encountered in clinical practice. There are three therapeutic goals to consider for patients with atrial fibrillation: restoration and maintenance of normal sinus rhythm, ventricular rate control and prevention of thromboembolism. The risks and benefits of each treatment must be considered for each patient. AMIODARONE Amiodarone is a drug with varying electrophysiological properties. § Prolongs action potential duration and refractory period of all cardiac tissue-potassium channel blockade (class III action). § Minor decrease in the upstroke velocity of phase 0 - Sodium channel blockade (class I action). § Noncompetitive β-receptor blockade (class II action). § Noncompetitive α-receptor blockade. § Calcium channel-blocking activity (class IV action). AMIODARONE FOR NORMAL SINUS RHYTHM RESTORATION Amiodarone could be used as a first choice drug, provided that rapid conversion is not required. For long lasting atrial fibrillation, amiodarone appears to be a very promising agent for sinus rhythm restoration, although long-term therapy is needed in order to achieve a high conversion rate. AMIODARONE FOR NORMAL SINUS RHYTHM MAINTENANCE Low-dose amiodarone is superior to placebo and can be used for the maintenance of sinus rhythm in patients with recurrent, symptomatic atrial fibrillation. Unfortunately, even in using the low dosages the dose-related side effects of long-term therapy were unavoidable AMIODARONE FOR HEART RATE CONTROL Amiodarone is effective for heart-rate control, in patients with permanent atrial fibrillation. It could therefore, be an alternative pharmaceutical therapy in heart failure cases.
