Abstract Metabolomics may serve as an innovative tool in clinical trials empowering new diagnostic and prognostic approaches. In the era of precision oncology, patient stratification optimization as well as decision making becomes crucial. For this, dissecting the molecular fingerprint of each patient drives companion biomarkers identification and validation toward companion diagnostics. Herein, nuclear magnetic resonance spectroscopy and liquid chromatography mass spectrometry, two state-of-the-art high-throughput technologies are explored as well as current developments and challenges in the context of companion diagnostics in clinical practice. Emphasis is put on clinical practice cases in precision oncology, while health policy and regulatory agencies’ demands are discussed. In conclusion, the cutting-edge technologies explored present great prospects for development in the field of metabolomics, especially in cancer, as current clinical studies are rather encouraging.
