Abstract PURPOSE: The purpose of our study is to perform and evaluate screening for ocular diseases using a non-mydriatic digital fundus camera as part of primary prevention for the first time in remote areas of central Crete METHODS: This study included random people regardless of sex, age and race of the general population of the wider region of Heraklion district ,who attended for screening at three spots within a non-mydriatic fundus camera (DRS, CenterVue Spa, Italy) .The sample was divided in 2 groups (A, B) according to the different field (upper or temporal) of the image capture. The photos were captured from each eye for 3 fields with common central and nasal areas. All images were stored in a network platform and after being evaluated by a retina specialist ophthalmologist, those who accepted the recommendation were examined in detail at ophthalmology clinic of University Hospital of Heraklion .Α statistical analysis of the general characteristics of the screening population was performed and after that followed analysis of the retinal abnormalities. Next was calculated the false positive screening findings. Last step was the evaluation of the service necessity by counting parameters like the last ophthalmological examination and risk factors for Diabetic Retinopathy (DR), Hypertensive Retinopathy (HR), Age Macular Degeneration (AMD) and Glaucoma. The time required to evaluate a patient and that which required conducting a complete screening session was recorded. RESULTS: A total of 1310 eyes were screened, 59.0% were women and 41.0% men., mean age of group A was 62.7 ± 13.1 and 65.7 ± 11.2 years for women and men, respectively, the mean of B group was 62.1 ± 12.6 and 63.9 ± 12.9 years. From all the participants 37.9% and 43.5% of women and men, respectively, presented abnormal findings during the assessment and a percentage of 5.5% and 10.6% of women and men pictures were inadequate for assessment. The frequencies for AMD, DR, optic nerve disorder (OND), and Others amounted to 31.7% , 11.7%, 35.4%, and 21.3%. Revaluation in University Hospital performed on 105 suspects. The mean Best Corrected Visual Acuity(BCVA) of the right eye was 0.06 ± 0.14 logMAR and 0.11 ± 0.31 logMAR for women and men, respectively, and the left eye 0.05 ± 0.07 logMAR and 0.08 ± 0.13.False positives diagnoses were 9.26%, 6.25%, 5.0%, 20.00% and 4.76% for AMD , DR, OND, OTHER, Epiretinal Membrane(ERM).The duration of screening for each examinee and the evaluation of a visit respectively on average for all regions stood at 10.34 ± 3.47 minutes and 1.54 ± 0.98 minutes. CONCLUSIONS: The participation of the local population was unexpected, which gave us the opportunity to have a wide sample for analysis. The increased mean age of those regions affected the mean age of the sample and hence the large incidence of AMD. The overall detection results of retina abnormalities and poor ocular medical examination showed and highlight the importance of the service in remote areas. The rate of retinal pathology suspicion was around 40% .OND represented the most frequent pathology but we must separate high from low risk suspects. The proportion of false positives for basic pathologies was quite low, below 10%.We conclude that 6 frequent ocular pathologies were satisfactory diagnosed from images with pupil diameter of more than 2.7mm, the evaluation time is low such us screening photo capture. Broad population screening is justified because of the high rate of asymptomatic retinal abnormalities. The ophthalmic community should lead these initiatives in order to promote ocular health.
