Abstract |
Demographic ageing and the evolution of biomedical technology in the
recent decades have on the one hand caused an explosion of demand for health
services, but also the cost of offering these services. In many countries there are
already in place scientific methods of calculating the costs of health services and
measures for cost containment. In Greece, hospitals and social insurance pension
funds currently operate with deficits of Billions of Euros, and their operation is
characterised by a total lack of both computerisation and application of cost
control methods.
Apart from the traditional methods of cost reimbursement of hospital
services, in recent years the logic of cost “packaging” has been applied, at least to
certain categories of surgical operations
The purpose of this essay is to approach the cost calculation of the widely
held surgical operation of laparoscopic cholecystectomy, end up with a cost
amount which will include all the relevant to this packaged operation expenses,
and compare this to the amount that social insurance pension funds currently
reimburse but also the tariffs that are in effect internationally.
Materials and methods: A regional and modern 200 bed private clinic with
fully computerised accounting systems was selected. Collection of data was made
via staff interviews, data provided by the Finance department and personnel
department of the said clinic.
Results: The 2 largest pension funds currently reimburse the amount of Euro
73.37 for surgery theatre costs and Euro 14.95 for sedation costs, while not
accounting for costs of materials but including some tests and medicines, the total
reimbursement cost amounts from 150€ (ΙΚΑ) up to 367€ (ΟΠΑΔ), where the
respective “package” cost amount for hip arthroplasty is 2054,29 €, excluding the
cost of operation materials.
Our own calculation, which includes the cost of materials, has resulted in a
final amount of 2315.4 Euros, 18% below the international average cost.
We suggest that the enactment of "surgical operations packages " will
ultimately function as a disincentive to artificial demand of health services
and towards cost containment, while allowing different options for the patients
and ultimately contributing towards them receiving better service .
