Abstract Introduction: Strengthening Primary Health Care (PHC) is one of the goals of the European Action Plan on integrated health services in Greece. The evaluation of the quality of health services is possible through the measurement of the experience and the expectations of the users from these services. The provision of PHC is carried out by 200 Health Centers (in urban and rural areas) and by 120 Local Health Units (TOMY) (in urban areas). Purpose: The purpose of this study is to investigate the experiences and expectations of patients for PHC services on the island of Karpathos in comparison with those of patients from Crete. The research questions concern the expectations and experiences of patients regarding the PHC services provided in areas with different characteristics, such as an isolated island, a semi-rural / urban and an urban area, as well as the strengthening of the existing literature to improve of PHC depending on local conditions. Methods: The research was based on the Quality and Costs of Primary Care in Europe (QUALICOPC). The respondents are 130 people (50 from the Health Center of Karpathos, 30 from the Health Center of Agia Varvara Heraklion and fifty from the 4th TOMY of Heraklion), who each evaluated their experience from the PHC services by completing the questionnaire experience QUALICOP (QUALICOPC-experiences), as well as their expectations in the QUALICOPC-patient values questionnaire (QUALICOPC-expectations). The questionnaires were randomly distributed and a total of 260 questionnaires were collected. Results: The analysis of the questionnaires revealed the satisfaction of the participants with their experience in all three PHC units under study, with the exception of the waiting time at the doctor's office which was longer in the Karpathos Health Center in relation to the two health structures of Heraklion. It is interesting that the expectations that the patients (visitors) from the Agia Varvara Health Center and the 4th TOMY have from the doctors are higher than those of the Karpathos Health Center. Conclusions: The data collected from this research could be used to improve the quality of PHC services in Heraklion and Karpathos, in the context of this reorganization into a service with a human-centered character. The present study revealed factors that patients (visitors) consider to be very important for their health and to which efforts to improve the PHC sector could be directed.
