Abstract Introduction: Community acquired pneumonia is one of the most common infections with high morbidity and mortality worldwide. Patients often bypass primary care services and visit hospitals. The purpose of this study was to investigate the reasons for patients non-visiting primary care and being directly addressed to hospitals. Methods: Our study was an extension of a cross-sectional study conducted in rural regions of Heraklion in order to estimate the prevalence of pneumococcal community-acquired pneumonia. In September 2012, all patients who were hospitalized in one of the two hospitals in the region of Heraklion participated in a telephone interview survey with open-and closed-ended questions designed to explore the causes that led them to contact directly the hospitals. Results: An important finding of our study was the fact that the majority of patients sought help from relatives or neighbours (23.3%) and that relatives-neighbours motivate them to ask for help (56.7%). Our studies revealed that a variety of factors determine the decision of the patients to visit the hospitals. Some of these factors were: comorbidities, previous hospital experience, reputation, distance, symptoms, specialists, the sense of security provided by the hospital and economic reasons. Patients mentioned several reasons for not visiting primary care services like; waste of time, lack of equipment, accessibility. Conclusions: The decision of a patient to visit or not a hospital unit depends on a set of predisposing and empowering factors as well as from factors from the health care system.
