Abstract Aim: To investigate the role of CT in assessing splenic lymphomatous involvement. Methodology: I. The CTs of 140 pts with normal spleen (group A) were evaluated in order to establish normative data of splenic volume (SV). II. Abominal CTs were performed in 58 pts with lymphoma before, during and after therapy (group B), a total of 282 CTs. Splenic lesions, SV, and other sites of involvement were recorded. Results: I. The mean value of SV was 214.6 cm3 (107.2-314.5) and did not vary significantly with patient's age, gender, weight or body index. II. Three groups of pts were identified: a)B1 (n=20) presented no changes in SV (p=0.34, r=0.09). All but one patient had normal SV and 30% showed other sites of involvement. b)B2(n=25) presented a decrease in SV (p<0.0001, r=-0.72) during therapy. Splenic and extrasplenic lesions, evident on pretreatment CT in 20% and 80% respectively, underwent remission at the same time. c)B3 (n=12) presented an increase in SV after therapy together with signs of disease recurrence. One of the 58 pts developed portal hypertension and was not classified. Conclusion: CT-assessed splenic volume may serve as an indicator of splenic involvement in the course of lymphomas.
