Abstract Identity is a central concept in substance abuse disorder as it is considered to be destroyed during use. Thus, in the process of rehabilitation, the reconstruction of identity is necessary for recovery. Users’ narratives play an important role in creating a new identity. Narration of life history is used by the 12-step groups in order their members to build a non-user identity and at the same time to offer a coherent sense of self. Although substitution opioid treatment is the most common proposal for detoxification, the combination of professional programs with 12-step groups shows increased success rates. The purpose of this research is to investigate the reconstruction of the identity of drug users that are members of a substitution program and at the same time participate in a 12-step group. Narrative biographical interviews were conducted with seven members of the group, followed by a narrative analysis of the data. Seven narrative units emerged: identity during substance abuse, causes that led to use, relationships with the family, previous attempts to stop using, entering the substitution program, the experience of the 12-step group and the attempt of identity reconstruction. The first five narrative sections highlight the users’ spoiled identity, while in the last two there is an attempt of identity reconstruction. Members feel that they belong to a new group and through the narratives they refer to the differences they observe between the previous and the current situation and describe the changes that have begun to take place.
