Abstract |
Introduction – aim: The low birthrate and the demographic ageing of population in
Greece caused the increase of demand of health services. The situation described
above led to intense reflection about the viability of health systems. The high levels of
sanitary expenses in Greece compared to the European Union, contributed to the
gradual collapse of systems of social insurance. Specifically, the National Health
System of Greece is characterized by the high level of private financing. Based on the
fact that the private sector has an important share of the market of obstetric services,
the objective of the present study is to approach how much the individuals pay for a
delivery that takes place in a private hospital. In particular, we will calculate the
average cost of a delivery that takes place in a private maternity hospital and the
money that family takes back from the social insurance as a compensation for the
hospitalisation in the private clinic.
Data and Methods: All available economic information from a modern private
hospital of municipality of Heraklion were analysed in order to estimate the
expenditures of obstetric care. Also, economic data were collected by the
organisations Social Insurance Institute (ΙΚΑ), Agricultural Insurance Institute
(ΟΓΑ), Public Employees Insurance Institute (ΟΠΑΔ) and Self-employing Insurance
Institute (ΟΑΕΕ) regarding the compensation for obstetric services by the social
insurance. Data were collected from the personnel of the private hospital mentioned
above using interviews. In addition, information were analysed with the support of the
accounts department and the department of computerization. Moreover, economic
data were gathered from the institutions of social insurance and three more private
hospitals, one in Crete and two in Athens. Important information was drawn from two
public hospitals of municipality of Heraklion and the registry office of the area.
Results: The expenses of hospitalisation in the case of childbirth in private clinic of
Crete range from 1.796,94 € to 2.374,52 €, while the charges for a caesarean section
range from 2.121,26 € to 2.790,96 €. The fee of the obstetrician constitutes additional
expense for the family. The charges in the case of childbirth in a private obstetric
home of Athens are 35% more than those in a clinic of Crete. The institution of social
insurance compensate the family for the delivery as follows: Social Insurance Institute
(ΙΚΑ) 991.20 € - Public Employees Insurance Institute (ΟΠΑΔ) 1.000 € -
Agricultural Insurance Institute 225.04 € - Self-employing Insurance Institute (ΟΑΕΕ)
800 €.
Conclusions: The social insurance is unable to cover the expenses for the delivery in
a private organisation. As a result, the cost of the delivery is a great strain on
individual’s resources. This fact combined with other social parameters aggravates the
problem of the low birthrate and the ageing of population in our country. A contract
between the institutions of social insurance and the private maternity clinics is
recommended as a measure of cleansing the financing of obstetrical services.
