Abstract |
Purpose: Τo analyze corneal topographies of relatives of patients with familial
keratoconus in order to investigate specific topographic patterns via colored maps and
keratoconic indices and to improve the diagnostic criteria to distinguish the suspect from
the normal corneas.
Patients and methods: A prospective study in which 39 families of keratoconus patients
participated, including 54 keratoconus patients, 34 first-degree relatives and a control
group consisting of 53 subjects, who had no family history for keratoconus and came to
our center for a preoperative examination in refractive surgery. Corneal topography
examination was performed on both eyes of participants with the Galilei Dual-
Scheimpflug Analyzer and corneal maps were analyzed. Each topography was analyzed
qualitatively through colored maps analyses, where the increment scale was at 0.5 diopter
and quantitatively using the specific indices that Galilei provides. Statistical analysis was
performed among the three groups: keratoconus patients, relatives and control according
to their corneal shape.
Results: The percentage of familial keratoconus in IVO is estimated around 25%.Two
corneal patterns were overexpressed in the relatives of familial keratoconus patients in
53% of all of the relatives. Quantitative analysis showed that 7 parameters were
statistically significant among suspect corneas and normal. Results of the statistical
analysis underline that IS (inferior-superior) index was the most reliable index το
distinguish between suspect and normal groups. Values of IS over than 0.82 D indicates
that the pattern of the cornea is suspect.
Conclusion: This study is the first one that analyses in a wide extent the corneal
topographies of familial keratoconus with the use of Dual Scheimpflug technology. The
findings in clearly established genetic keratoconus allowed to detect suspect corneal
patterns and bring new evidence in order to diagnose forme fruste keratoconus. Galilei has the ability to detect suspect profiles and to differentiate them from normal with some
but not all of the indices that provides.
