Abstract BACKGROUND: Patients with Chronic Lymphocytic Leukemia (CLL) are at risk of infectious morbidity and mortality.In early, untreated CLL patients, this risk may be related to hypogammaglobulinemia, but in patients with advanced disease, particularly those treated with the newer purine analogues, neutropenia and defects in cell-mediated immunity appear to be the major predisposing factors. Fludarabine, a novel purine analogue with major activity in CLL, has resulted in serious and unusual opportunistic infections, according to some brief reports. However, systemic analysis of the observed Fludarabine-associated infectious morbidity is lacking. The scope of the study was to review the changing pattern of infections in patients with CLL treated with Fludarabine. PATIENTS AND METHODS: All patients with CLL treated with Fludarabine cared for at MD Anderson Cancer Center between 1984 and 1991 were analyzed retrospectively. Patients were included in the study if they had advanced CLL and had received a 5 day course of Fludarabine (30mg/m2/day) with or without prednisone (30mg/m2/day) on a monthly basis.The significance of their febrile episodes was determined by recording and analyzing each one, by isolation of the causative organism and evaluation of the outcome. RESULTS: Infections of all types occured more frequently in patients pretreated with cytotoxic chemotherapy (47%) than in untreated patients (24%)(p<.001). Three hundred eighty-two infectious episodes occured in 212 of the 402 CLL patients. Among them, 284 were documented infections and 98 fevers of unknown origin.
