Alternative Title Assessment of auditory disorders and tinnitus under the influence of high levels of anxiety

Abstract Tinnitus is one of the most common symptoms in patients with hearing disorders. However, therapy or suppression techniques are still at an exploratory stage. This thesis aimed at exploring a) the influence of high of anxiety on tinnitus sufferers and b) the personality characteristics of tinnitus patients. No such research has been carried out in a Greek population before. The Laboratory of Audiology and Neurotology of the ENT Clinic of the University Hospital, Heraklion Crete carried out a first attempt to record epidemiological characteristics of tinnitus patients, along with a proposal of a tinnitus suppression method. The research reported here was a three-year study of 100 volunteers who were tinnitus patients of both sexes, from 18 to 65 years of age. The research protocol, which included a complete otorhinolaryngological and audiological assessment, clinical interview and administration of Minnesota Multiphasic Personality Inventory (MMPI), was applied to all patients. The factors investigated were the duration of tinnitus, the subjective assessment of tinnitus intensity, the hearing impairment, age and sex and their correlation with personality characteristics as assessed by the MMPI. We also investigated the coping techniques individuals employ to confront the symptom. The statistical package used for data analyses was the SPSS. Our findings we have led to the following conclusions. 1. Patients who had been suffering from tinnitus for longer than five years tended to adopt the self-image of being physically handicapped as well as passive or negative coping techniques. 2. Tinnitus intensity varied according to patients' sex, age and psychological profile. Patients under high levels of anxiety or dysthymia tended to evaluate their tinnitus as more intense. 3. Male patients, compared to female patients evaluated their symptom as less intense, however they seemed to be more annoyed be it and were pathologically preoccupied with their health. They also came up with a higher hypochondriasis level compared to women. Women overreacted to the symptom, described it as less annoying and proved to be suspicious to the progress of their condition. They were also less reliable than men, as far as their MMPI answers were concerned. 4. The hearing impairment proved to be a decisive factor, especially when it was combined with long lasting tinnitus. Patients had an attitude of disbelief and suspicion concerning their environment and their physical problem became central in their lives. 5. Older patients were better adjusted and less anxious or depressive compared to younger patients, who tended to become socially isolated and seemed to be more pessimistic. 6. The individuals' coping techniques were directly influenced by their personality structure, the possible psychopathology background they have had before the tinnitus, the quality and quantity of information on their condition that was given to them by the therapeutic team at the initial phase of their symptom and, finally, by individual developmental factors. 7. The personality characteristics and psychological profile of the individuals was assessed as being particularly important concerning their adjustment to the symptom, as well as their co-operation and dealing with the therapeutic team.
