Abstract Background A significant percentage of patients after a Roux-en-Y reconstruction complain of symptoms suggesting food stasis as a result of dysmotility of either the gastric remnant or/and the efferent jejunal limb. The aim of the study was to investigate the possible effect of i.v. erythromycin on gastric emptying of solids in patients with a Roux-en-Y procedure. Patients 24 patients with a Roux-en-Y procedure participated in the study. 10 of them had had truncal vagotomy with pyloroplasty (TVP), while the remaining 14 had had a Billroth II subtotal gastrectomy (Bill II), as the initial antiulcer procedure respectively. 16 healthy subjects served as controls. Method In a double blind controlled fashion, all healthy subjects and patients had the gastric emptying of a standard radiolabelled solid meal assessed after giving placedo or 200mg of erythromycin i.v. The subjects were scanned by gamma camera and the emptying curves were constructed. From these curves the half-time of gastric emptying (T1/2) was calculated. Results Patients with severe symptoms of gastric statis had a significantly longer T1/2 than patients with mild or no symptoms (P =0.002). Patients with a Bill II as the initial antiulcer procedure had a significantly worse grading of symptoms (P=0.01) and a significantly prolonged T 1/2 (P=0.016) as compared to patients with a TVP as the initial untiulcer procedure. Erythromycin significantly reduced the T 1/2 in the controls (P=0.0008) and all patients after Roux-en-Y procedure (P=0.000003).Conclusion. Erythromycin could be proven a useful (abstract truncated)
