Abstract Despite the continuous improvement of therapeutic protocols, as well as supportive care, for the treatment of malignancies in children and adolescents, many patients and physicians often deal with early and late complications. These include suppression and disfunction of the immune system. Most of the literature consists of studies based on acute lymphoblastic leukemia (ALL). The present retrospective study aims to record and analyze the immune reconstitution following cancer treatment. Specifically, data was from children, adolescents and young adults, who were treated for cancer during the period from 01/2015 to 12/2019. Demographic data and data regarding immune status were isolated and analyzed (at four time points: end of treatment, and 2, 6 and 12 months after completion) from a total of 65 patients, aged 5 months to 19 years. Patients were further categorized into four subpopulations (acute lymphoblastic leukemia, solid tumors, lymphomas, acute myeloid leukemia/chronic myeloid leukemia). Also, treatment parameters such as risk stratification, radiation and immunotherapy were included. The majority of the patients developed some level of impairment at the end of treatment. Lymphocytes and immunoglobulin M (IgM) were affected more and for a longer period, with further deterioration among the patients that underwent radiotherapy. The most severe and long-term complications of the immune system were observed in patients with lymphoma, while patients with ALL showed the fastest recovery. Ultimately, the majority of patients experience immune reconstitution within 2 to 6 months after completing treatment. The study indicates that different diagnosis, consequently treatment, have different effect on immunity with different recovery tendency.
