Abstract Biliary obstruction and subsequent obstructive jaundice, comprise common but particularly complex clinical situations in cases of biliary and pancreatic malignancies. Treatment of malignant biliary obstruction depends on the site and the stage of malignant disease and contributes in total therapeutic approach affecting postoperative period, adverse events, quality of life and survival of the patients. Therapeutic endoscopy has incredibly developed since 1975, when it was firstly introduced. Endoscopic Retrograde Cholangiopancreatography (ERCP) plays a crucial role in non – surgical treatment of various diseases of the pancreas and the biliary tract, especially in malignant biliary obstruction with the use of biliary stenting either preoperatively or during palliative treatment. The success of stent placement, stent patency, re-intervention rates, quality of life and total survival of the patients can be determined by a number of factors including stent type, material as well as insertion technique. Recently, novel technical modalities have been developed, including Percutaneous Transhepatic Biliary Drainage (PTBD), Endoscopic Ultrasound Biliary Drainage (EUS – BD), Photodynamic Therapy (PDT) and use of Radiofrequency Ablation (RFA).
