Abstract Forty-eight patients with primary hyperparathyroidism were studied preoperatively with US (48 patients), with CT (38 patients) and with MRI (25 patients). The overall diagnostic accuracy for US was 87%, for CT 66% and for MRI 92%. In the case of patients with a single parathyroid adenoma, US was the most cost-effective localization technique with a detection rate of 95%. CT had a lower detection rate (74%) but was of particular value for fairly large ectopic adenomas in the root of the neck. MRI had a high detection rate (91%) but was quite expensive. In patients with multiple gland disease (primary hyperplasia or multiple adenomas), no single study alone was sufficient. However, the combination of the above three studies suggested the presence of disease in more than one gland in 78%. US, CT and MR imaging followed by surgery by an experienced surgeon provided excellent results in the case of all 48 patients with primary hyperparathyroidism.
