Abstract Introduction: Smoking during pregnancy is still a challenge for public health. A series of factors have been found to affect smoking behavior during pregnancy; yet of notable research interest is the relation of smoking during pregnancy with the social background of the mother, especially after acknowledging the social factors in interpretation and justification of various results and constants concerning health. Methodology: The present research is conducted within a prospective cohort Mother-Child study (Rhea Study) (n=550). Questionnaires of the 12th and 30th week of pregnancy were used, while the measurement of the mother’s social capital was performed using the Social Capital Questionnaire (SCQ-G) during the 24th week. Multiple logistic regression was used to evaluate the relation of the mother’s social capital with the smoking behavior during pregnancy, after checking for possible confounding factors. Results: Medium participation levels to the local community (between the 10th and 90th percentile of observations) were strongly associated with smoking during pregnancy, after checking for confounding factors (OR 2.73, 95%CI= 1.18-6.27, p= 0.02). Furthermore, for every increase by a unit to the social capital feelings of safety, there was a 12% increase to the probability of smoking behavior during pregnancy, after checking for confounding factors (Adjusted OR 1.12 95% CI= 0.97-1.30), even though this assessment was not statistically important. Conclusion: Results of the present study suggest that participation to the community may be associated with an increased probability of smoking during pregnancy. Future studies are required to distinguish the various types of participation and factors of social capital, so to better understand their relation with the smoking behavior during pregnancy.
