Abstract Historically, radiologists have been communicating the results of diagnostic imaging examinations to the referring/ treating physician – therefore for years they remained "invisible". Their role today is multifaceted and includes the identification of pathological findings in imaging examinations, the diagnostic interpretation of these findings as well as the communication of the results for further treatment or imaging re-examination, and even the provision of treatment through image-guided intervention. Although radiologists have variable degrees of direct contact with patients, they have duties towards them and are bound by the principles of medical ethics. Τhe following unique to diagnostic radiology practice ethical concerns are recognized in literature: overuse of diagnostic imaging / useless imaging tests, missed findings/ wrong diagnoses, remote diagnosis (teleradiology), applications of artificial intelligence in medical imaging and issues of communication and trust between radiologists and patients. Related research in Greece is non-existent. This paper attempts, through empirical research, to understand the ethically problematic issues radiologists face during clinical practice in the Greek National Health System and how they deal with them and to discuss them in the light of the four basic principles of bioethics as expressed by Beauchamp and Childress - respect for autonomy, benefit/benevolence/charity, non-harm and justice - in order to examine the conditions for the practice of this specialty for optimal quality health services to patients. For diagnostic radiology to maintain the character of a medical art in the service of man, radiologists need to practice their duties embracing a principle-based approach. The reported ethical concerns appear to be consistent with those reported over time in the international literature and may be adequately addressed by the development of evidence-based guidelines of good clinical practice, legally enshrined at the national level, and focused education. Further research in this direction is required.
