Abstract Background: The COVID-19 pandemic is the subject of numerous studies since its emergence, both for its immediate consequences during illness and its clinical presentation that remain for a long time after the initial infection. Thus, a new entity arises, the Post-COVID syndrome, which describes the persistent symptoms that occur weeks or months after the acute phase of the disease, without being able to attribute it to an alternative diagnosis. It appears three to four weeks after the infection, persists for at least two months, and can affect all systems of the organism. The diversity in clinical manifestations affects the quality of life of patients and reduces their functionality, making it difficult for them to return to their daily routine. The reduction of cognitive functions that is often observed in COVID-19 patients is of particular interest. Objective: The main objective of this research is to investigate the post-COVID impact on the quality of life and cognitive functions of COVID-19 patients who were treated at home or in a COVID clinic, including correlation with the severity of the illness, vaccine coverage (complete, partial, or none), and the presence of comorbidities. Methods: The study is a retrospective analysis of patients who were followed in the "post-COVID" clinic at the Venizelio General Hospital of Heraklion-Crete,Greece after being infected with COVID-19 from June to December 2022. The quality of life of patients was evaluated using the EQ-5D score (EQ-5D-5L,EQ-VAS), and their cognitive function was assessed using the Mini Mental State Examination (MMSE) within one to three months after illness. The EQ-5D is a general measure of quality of life that consists of two tools: a descriptive system (EQ-5D-5L) and a visual analogue scale (EQ-VAS). The MMSE is a widely used test of cognitive function that includes tests of orientation, attention, memory, language, and visual-spatial perceptions. Demographic information such as age, gender, occupation, location, underlying health conditions, date of illness, and date of admission to the post-COVID clinic was recorded and analyzed, as well as clinical and laboratory information such as type of oxygen therapy, complications, length of hospitalization, duration of nursing (COVID clinic or ICU). Criteria for exclusion were prior mobility problems, psychiatric disorders and cognitive deficiencies. The collected data were recorded electronically in a Microsoft Excel database. Data analysis was performed using appropriate software (SPSS 29). Results: In this study, 250 patients were included, the majority of whom reported post-COVID symptoms up to the eighth month of re-evaluation (70.1-88.3%). 65.2% of the participants had comorbidities. Patients with comorbidities had a more severe illness and a need for hospitalization in a COVID Clinic or ICU (32.5% and 3.1% respectively), as well as more than 50% of the ventilated patients. The mean value on the health quality scale EQ-VAS for the first month was not different or significantly low in patients with serious or critical illness (70%) and those hospitalized in ICU or COVID Clinic (70%). Significant unequal distribution was observed in the evaluation of quality of life using the EQ-5D-5L questionnaire in the different subgroups, with the more severe problems being reported by the female gender and participants with comorbidities and mostly affecting the presence of symptoms as pain/discomfort and stress/distress. MMSE score had pathological results concerning orientation, concentration, recall, and language among the patients with pathological scores (<25) compared to patients with higher scores. Immediate memory was not affected and did not differ between the two groups. The results were affected by the increase in the severity of the illness and the need of hospitalization in COVID clinic or ICU department. The EQ-VAS and EQ-5D-5L score were independently associated with the presence of post-COVID symptoms after 1 month of the infection. Conclusion: The presence of comorbidities and the presence of post-COVID symptoms after 1 month, severe or critical illness and the need for advanced nursing care are predictors of poor quality of life and cognitive dysfunction after COVID-19 illness.
