Abstract |
Refractive error correction has been the main focus of the research community in the recent years because of the increasing need for best possible achieved uncorrected visual acuity. The main focus of this work is the development of an innovative technique for correcting any corneal irregularity and is easily customizable from patient to patient.
Fresh porcine eyes are used for our experiments as well as model eyes for surgery practicing. We perform the operation using a femto-second laser to create the two lamellar incisions needed to form a customized intrastromal lenticule. These incisions are designed to intersect in the periphery thus creating the lenticule.. This is achieved through the application of a proprietary photo-responsive polymeric gel and a semisoft contact lens on the cornea. The complex of contact lens, gel and cornea is exposed to UV radiation which causes the gel’s polymerization. This artificial new surface of the cornea is used as a referral for the first femto-laser incision in order to replicate an intrastromal mirror incision of desired curvature that will be the posterior surface of the lenticule.
At that point a commercial iFs (Johnson & Johnson) cone is used for the applanation of the cornea-gel complex to generate the first intrastromal cut. After that the contact lens and the gel are removed once the first incision is completed. To generate the intrastromal lenticule that includes our desired refractive correction, this time we applanate the natural surface of the cornea and perform the second lamellar cut alongside with a small side cut from where the lenticule is extracted.
Lastly, OCT and Pentacam are utilized to analyze the differences of the cornea before and after the procedure.
