Abstract Introduction: Pain, as one of the symptoms of patients with hematological malignancies (HM), is attributed both to the disease itself and to the side effects of treatments. A major problem of patients with HM is that pain is not properly assessed, resulting in delayed diagnosis and ineffective treatment. Aim: The investigation of pain (evaluation and management) in patients with HM, both during the active phase of the disease and in survival. Method: A systematic literature review was conducted between the years 2012–2022 in Pubmed, with the keywords: pain; hematologic malignancies; management; assessment; multiple myeloma; lymphoma; leukemia; hematopoietic stem cell transplantation. Inclusion criteria were: studies in English; with study population of adults (>19 years) with HM and referred to pain assessment and/or management. Studies involving mixed populations of hematological and solid tumor malignancies and/or mixed populations of adults and children as well as literature and systematic reviews were excluded. Results: The literature search identified thirty-seven studies that met all inclusion criteria. Sixteen studies included patients undergoing hematopoietic stem cell transplantation and twenty involved multiple myeloma patient. The remaining studies involved mixed populations with HM. Five studies referred to methods and interventions for pain management, such as the application of acupuncture, music therapy, foot massage and the application of mouthwash in oral mycositis. Also two studies focused on pain associated with peripheral neuropathy, while four studies on pain associated with oral mycositis. Twenty-six studies evaluated patients' quality of life, fifteen of which involved patients with multiple myeloma. The studies used a large number of measurement tools to assess pain, symptoms, quality of life and functioning. Conclusions: Studies are extremely heterogeneous. The pain experienced by patients with ΗΜ is underestimated and therefore undertreated. The systematic evaluation and recording of pain by healthcare professionals, using appropriate tools and its correct management will significantly contribute to the understanding of the pain experience.
