Abstract Introduction: While progress has been made in promoting patient autonomy, there are still gaps in care coordination and shared understanding of care goals, including the advance establishment of care guidelines by clinicians as early as the diagnosis of a life-threatening illness. Aim: The aim of the review is to explore the care goals near the end of life, set by patients in collaboration with healthcare professionals. Methods: A systematic review of the literature in scientific databases will be carried out with suggested keywords: end of life, care, goals of care, quality of care, palliative care. Results: The literature review showed that good communication between patient and health care professional and early and joint definition of goals of care, already in the early stages of a terminal illness, can reduce the use of additional treatments aimed at preserving life, reduce conflicts with the family environment, improve patients' own satisfaction and prepare their loved ones for the upcoming bereavement. An obstacle to this collaboration is that many health professionals feel ill-equipped to engage in discussions about end-of-life care as there is a perception that hope will be eliminated, whereas instead such conversations can allow people to express what their goals are. Conclusion: End-of-life care refers to the assessment and satisfaction of the physical, spiritual and psychosocial needs of patients and their caregivers and is provided by health professionals and support staff. In this context, health professionals should be trained in the principles of palliative care and in effective communication with patients in order to respect any different goals set by patients and their carers. Early discussion of care goals and ongoing communication between healthcare professionals and patients about any change in goals ensures patient satisfaction and facilitates the grieving process for caregivers.
