Abstract Malignant gliomas constitute a complex disease phenotype that demands optimum decision-making. Despite being the most common type of primary brain tumors, gliomas are highly heterogeneous when their pathophysiology and response to treatment are considered. Such inter-individual variability also renders optimum patient stratification extremely difficult. Recent evidence highlight that the gene-environment interplay becomes of fundamental importance in oncogenesis and progression of gliomas. For glioblastoma multiforme, in particular, genotype-to-phenotype associations are critical. Herein, we put emphasis on a microRNA signature (hsa-miR-20a, hsa-miR-21, hsa-miR-10a) to map the inter-individual variability of glioblastoma multiforme patients toward optimum patient stratification. Data and text mining reported a key role for hsa-miR-20a, hsa-miR-21 and hsa-miR-10a in the regulation of gene expression in glioblastoma pathophysiology. Next, immunohistochemistry analysis of FFPE tissue samples of glioblastoma multiforme patients (n=37) was performed for TGFb-RII, hMSH2 and EphA8. microRNA expression levels were determined by RT-qPCR. hsa-miR-20a, hsa-miR-21 and hsa-miR-10a were up-regulated. Such findings are further supported by our immunohistochemistry analysis for TGFb-RII, hMSH2 and EphA8. Molecular-clinical correlations were drawn to classify high- vs. low-risk patients, shedding light to the molecular mechanisms involved in disease progression. This microRNA signature (hsa-miR-20a, hsa-miR-21, hsa-miR-10a) may further empower evidence-informed decision-making in clinical practice.
