Abstract |
Many bioethical issues are closely linked to family dynamics. A typical example is genetic tests, which provide important information not only for the proband but also for those connected with him by blood ties. This dual nature of genetic information, both familial and strictly personal, creates ethical dilemmas in its handling. On the one hand, the personal and sensitive nature of genetic data requires careful handling with maximum respect for the privacy and confidentiality of information, principles that are both grounded on the concept of autonomy. However, in cases where the genetic test indicates serious harm and there are treatment or preventive measure available, a duty of beneficence to warn relatives for the foreseeable harm arises. Although the majority of people who undergo a genetic test understand the importance of informing their biological relatives and consent to it, the quality and quantity of information that is eventually disseminated to relatives reveals the existence of barriers to its transmission, which are associated mainly with difficulties in family communication and family relationships. Genetic counseling, as an integral part of genetic testing, is the factor that makes information valuable, for both the patient and his family. The rapid development in the field of genetic testing creates an urgent need to develop ethically sound guidelines regarding genetic information disclosure. The moral dilemma that arises when the principles of autonomy and beneficence are in conflict, cannot be satisfactorily addressed using the Beauchamp and Childress system of principles. On the contrary, within the Kantian framework it is possible to find balance. According to Kant, autonomy is the freedom of the will to obey in moral principles, which in order to be moral should be universally applicable. Based on the Kantian approach, the duty of beneficence is contained in the concept of autonomy.
The role of genetic counseling is crucial, both in diagnosing and dealing with family difficulties that impede the flow of information. A systemic approach, on which family therapy is mainly based, is a valuable guide for investigating any issue related to family complexity. Genetic counseling can effectively assist the probands in trying to defend their autonomy, not individual but principled autonomy, and to help them effectively meet their moral obligations to their relatives through a systemic approach, which effectively integrates the psychosocial dimension into medical care.
