Abstract

The doctoral thesis entitled "EVALUATION OF PERCUTANEOUS TRANSLUMINAL ANGIOPLASTY OUTCOME IN PATIENTS WITH PERIPHERAL ARTERIAL DISEASE USING MODERN MEDICAL IMAGING TECHNIQUES (CT AND MR PERFUSION)" evaluates the results of percutaneous transluminal angioplasty (PTA) in patients with severe peripheral arterial disease using modern medical imaging techniques and particularly CT and MR perfusion techniques. Selected patients underwent CT and MR perfusion examinations with appropriate imaging protocols included sequences with (DCE-MRI) or without contrast administration (DW-MRI) before and after PTA. Moreover, the medical history, the predisposing factors and the clinical laboratory parameters of patients were recorded. The studies showed a significant improvement in MR / CT perfusion parameters after PTA. As far as CT perfusion technique, a correlation between CT perfusion parameters values and the clinical outcome of patients after revascularization was found. Moreover, further analysis of patients' MR examinations was performed and a correlation between perfusion parameters of DCE-MRI and DW-MRI was observed. Finally, an innovative technique of calculating limb perfusion using MRI without paramagnetic contrast (IVIM MRI) was tested and it was found to effectively show the results of PTA in patients with critical ischemia and highlight the differences in perfusion parameters between patients with peripheral arterial disease and healthy volunteers. CT perfusion, DCE-MRI and DW-MRI are reproducible techniques which can be useful tools for evaluation of lower limb hypoperfusion and estimation of PTA outcome in patients with CLI. The information provided by these examinations may aid reliable prediction of poor response to revascularization or an imminent amputation in patients with CLI. Moreover, they could be promising tools for monitoring of endovascular or surgical treatment and planning of revascularization strategy.
