Abstract Deep brain stimulation (DBS) is an invasive neurosurgical method. Though an electrode implantation and electrical stimulation brain areas, it is applied in cases of neurological and psychiatric diseases that do not respond to medication, producing positive results (reduction of symptoms and improvement of the quality of life). Because it interferes with the brain function, responsible for mental abilities and emotions, it is proposed that DBS can affect and change personality. Data support the postoperative appearance of changes in patients' behavior and psychological trends. Neuroethical studies of these changes have not led to a generally accepted view on the impact of DBS on personality. We investigated the reasons that prevent a common view and attributed them to both practical issues (how to measure the changes) and theoretical (related to concepts such as personality, personal identity and person). Clarifying the above concepts, we conclude that DBS cannot threaten the regulatory concepts of person and personality. It is possible to change the empirical meaning of personal identity, undermining an important moral capacity of the person, the agency. Therefore, the ethical concern shifts to the risk of agency, but in patients who resort to DBS this is already deeply compromised due to the existing disorder. In conclusion, the use of DBS should not raise ethical concerns because properly applied, it meets the principles of bioethics, as any biomedical achievement ought to, and may ultimately contribute to enhancing agency and its long-term realization.
