Abstract Introduction: The treatment of critical ill patients in the emergency department (ED) is a challenge as their needs are complex and their survival depends on many factors. Aim: the recording and the outcome of the critical ill patients entering the Emergency Department (TEP) of PAGNI over a specific period of time and their comparison with international practices and data. Methods: This is a prospective, descriptive study with a sample of convenience. Entry criteria were patients who were entered into the ED of PAGNI from: October 2018 - February 2019 and belonged to category 1 & 2 of the Emergency Severity Index (E.S.I) a tool for the priority of patients in the Triage system. Exclusion criteria were age &le; 18 years and lack of basic data for the research from their hospital admission card. We correlate the cause of visiting the ER, their length of stay (EDLOS) and the outcome as far as the 48 hour outcome of these patients. The IBM SPSS 20.0 was used to analyze study data. Results: During the study, 160 patients were classified as ESI - 1 and ESI - 2. 160 patients were included in the study, of whom 106 (66.3%) were men. The mean age of the patients was 67.92 years. The three main reasons for these patients access to ED were problems from cardiovascular (41.3%), respiratory (36.3%) and trauma (4.4%, 4.4%, 1.3%) and other (12.5%). Their outcome in the ED of PAGNI was: admission to the ICU (60.0%), admission to common wards (24.4%), death in the ED (8.1%) and transfer to the surgery room 7.5%. The mean length of stay (EDLOS) of these patients in the ER was 2.37 hours. Their median length of stay (LOS) was 10.79 ± 8.65 days. Statistically significant was the length of stay in ER - their outcome and the relationship between the degree of significance of Glasgow Coma Scale (G.C.S.) and their outcome in the ER (Spearman rs = 0.395, p <0.001). Conclusion: The present study shows a generally satisfactory picture of the management of the critical ill patients in the ED of PAGNI: improvement in ED to 24.4%, rapid admissions from ED to ICU to 60%, (length of stay in the ED 2,37 hours) and most common 48 hours outcome was the transfer / stay in ICU /HCU (51% ), hospitalization in common ward (42.2%) and death (6.8%).The causes of death and length of stay in patients who died in ED (8.1%) need to be further investigated and analyzed.
