Abstract |
ABSTRACT Background Smoking is the main cause of preventable morbidity and mortality. Smoking cessation is a major public health issue today. General practitioners working in primary health care can play an important role in assisting their patients quit smoking. Objectives The aim of this systematic review was to assess the effectiveness of non pharmaceutical interventions in general practice for smoking cessation Search strategy The Pubmed database and Cochrane Central Register of Controlled Trials were searched. Date of the most recent search: August 2010 Selection criteria Randomized controlled trials of non pharmaceutical interventions for smoking cessation delivered by general practitioners in primary health care setting. Data collection and analysis Data was extracted in duplicate by two independent reviewers. The differences were settled after discussion and any disagreements were resolved by referral to a third reviewer. Studies that met the inclusion criteria were included in the review, their quality was assessed and the study population, the interventions delivered, the outcomes and the effectiveness of outcomes were documented. Studies included in the review were then categorized according to the type of intervention and the effectiveness of each intervention was assessed separately. Main results
25 trials were identified, conducted between 1972 and 2009. All trials assessed the effectiveness of interventions delivered by general practitioners in primary health care setting. The most effective interventions that gave statistically significant results were the motivational interviewing,

interventions based on the transtheoretical model of stages of change and intensive (structured) interventions. Simple advice, follow up visits, computer tailored letters and printed self help material showed no statistically significant outcomes compared to usual practice. Spirometry showed an increased effectiveness in smoking cessation that didn‘t reach statistical significance, except when spirometry results are announced as ―lung age‖. Authors’ conclusions Interventions based on motivational interviewing, the stages of change model and intensive-structured ones appear to be effective in helping patients quit smoking in primary health care. There is insufficient evidence to support the effectiveness of spirometry in smoking cessation.
