Abstract Introduction: Coronary Heart Disease (CHD) is ordinarily accompanied by low Omega-3 fatty acids status and risk of depression. Methods: A “case-control” observational study of 120 participants in Crete;60 experienced new onset CHD with an Acute Coronary Syndrome (ACS) and 60 screening individuals, men 35-75 years old and post menopause women to 75 years old, all free of depression history. The subcutaneous adipose tissue composition was examined by buttock autopsy and analyzed by gas chromatography. Two self-reported scales were used to estimate depression: the Beck Depression Inventory (BDI) and the Zung Self-Rating Depression Scale (ZSRDS). Results: ACS participants’ Docosahexaenoic acid (DHA) was inversely associated with BDI scale as determined by rho -0.306 Spearman's correlation (p΄&λτ0.05). Controls’ Polyunsaturated fatty acids (PUFA) were inversely associated with ZSRDS scale as showed by rho-0.301 (p΄&λτ0.05); likewise n-6 were inversely associated with ZSRDS scale as shown by rho- 0.305 (p ΄&λτ0, 05). Buttock adipose tissue amongst cases and controls differed in Mono Unsaturated Fatty Acids (MUFA) (p=0.008) and n-6 (p=0.002) composition and n-6/n-3 ratio (p=0.001); ACS participants depressive scores were higher than controls, as determined by BDI (p=0.032) and ZSRDS (p=0.004) scales respectively. Conclusions: These results replicate that dietary omega-3 fatty acids may provide anti-inflammatory protection to individuals undergoing non-fatal CHD additionally to depressive symptoms reduction; on the contrary, high omega-6 fatty acids composition may demonstrate pro-inflammatory effect, thus leading to adverse coronary events. Secondary prevention programs based on Mediterranean diet adherence could result in better coronary health.
