Title Η συμβολή του διορθικού υπερηχοτομογραφήματος στη μελέτη της σύστασης και του μεγέθους του προστάτη στην καλοήθη υπερπλασία του αδένα

Abstract The efficacy of suprabic (SU) an transrectal (TU) ultrasonography in accessing the size and the texture of the prostate in benign prostatic hyperplasia (BPH) is evaluated in the current work. Digital rectal examination, urethrocystography or endoscopy con not reliably assess the volume of the prostate and of the transitional zone of the prostate (TZP). Study population consisted of 105 patients 47-85 years old (mean 69,7 years) with clinically diagnodes BPH. Measurements of the anteroposterior transverse and craniocaudal diameters of the prostate and of TZP were performed on both SU and TU. Subsequently, the volume of the prostate and the TZP were calculated for both methods and the weight of the prostate and adenoma were estimated. The mean volume of the prostate was 47,8 +/- 20,0 cm^3 (ranging 21,0 - 123,0 cm^3) on TU and 50 +/- 22,2 cm^3 (ranging 18,8 - 133,2 cm^3) on SU. a strong correlation existed between suprapubically and transrectally performed volume measurements (r=0,948, p<0,001). the borders of the prostate were clearly delineated by both TU and SU in all patients. The perimeter of the TZP was identified in 93 with TU and in 84 with SU. The mean volume of the TZP was 22,5 +/- 16,5 cm^3 (ranging 5,3-92 cm^3)on TU and 23,7 +/- 17,1 cm^3 (ranging 5,8-108 cm^3) on SU. Volume measurements with SU correlated with those obtained with TU (r=0,953, p<0,001). The weight of the surgically removed adenoma 3,1 - 61,4 gr, mean value 19,7) did not differed (p<0,001) from the estimated TZP weight on ultrasonography (5-70 gr, mean value 20,9). No relationship was found between the texture of the TZP on TU and the histological subtypes of the removed adenomas. Suprapubic ultrasonography is a quick, convenient and cheep method that is effective in the evaluation of both prostatic and TZP volume. It may replace TU in the preoperative evaluation of conservatively treated patients with BPH, where consequtive measurements of the adenoma are required. TU is suggested when a biopsy is scheduled in patients with suspected malignancy.
