Abstract |
Introduction: Health-Care Associated Infections (HCAIs) are a serious problem for
health services. Proper hand hygiene contributes to reducing transmission of
pathogens and has been recognized as the most important measure to prevent the
transmission of health care associated infections. However, it has been demonstrated a
strong association between low compliance of health workers with hand hygiene and
high prevalence of HCAIs. Knowledge evaluation regarding hand hygiene among
nursing staff is an important step in promoting new cultural practices and guidelines
and thus their proper implementation.
Aim: To evaluate the compliance and knowledge of hand hygiene at the Nursing Staff
of the University Hospital of Heraklion, using a questionnaire. The ultimate goal of
this study was to enhance hand hygiene practices and design implementable strategies
to reduce transmission of pathogenic microorganisms served by HCWs’ hands.
Material and Methods: A cross-sectional survey was conducted among employees
of the Nursing Office of the Hospital, of which 288 were registered nurses, 129
auxiliary nurses and 35 midwives. The 83.7% were women. The method used for
employee knowledge assessment was based on an anonymous, self-completed
questionnaire based on WHO’s questionnaire “Hand Hygiene Knowledge
Questionnaire for Health-Care Workers”. The questions were based on its guidelines
evaluating all five key points of hand hygiene.
Results: The percentage of knowledge about hand hygiene was 52%. Although the
majority uses the ABHR as a routine (91.7%), only 50.3% knew the minimum time
required for its application. The level of knowledge for the most frequent source of
pathogens responsible for healthcare-associated infections was very low (26.3%),
while for the main route of cross-transmission of potentially harmful bacteria was
good (78.4%). A very high percentage was aware of the hand hygiene actions that
prevent transmission of germs to the patient (95.9% before touching a patient and
83.9% before an aseptic procedure) and the hand hygiene actions that prevent
transmission of germs to HCWs (92% after touching a patient and 93.6% after a risk of body fluid exposure). 54.9% considered hand rubbing causes skin dryness, 66.8%
that it is not more effective than hand washing and only 20.3% knew that the two
methods are not recommended to be performed in sequence. Most of the respondents
(>90%) knew that jewellery, damaged skin and artificial nails are factors associated
with an increased likehood of colonization of hands with pathogens.
Conclusions: Health care workers' knowledge of hand hygiene is considered as
moderate. Our findings are similar to these in other studies. It is important to
emphasize on continuing training and education of health workers on new guidelines
on hand hygiene and its relation to HCAIs, as well as on the quality of nursing
schools education.
