Abstract The widespread concern about the unfairness of inequalities in population health calls for the shift of ethical concerns to critical inquiry and inequality-based normative questions in the context of the socially and politically enriched ethics, constituted by bioethics. Ethical reflection on health extends beyond individual, health care and health research to include the ethics of social determinants of health. This raises a wide range of ethical questions and practical issues concerning inequalities in health that are involved in intervention policies to eliminate or reduce them. Since equality is a multidimensional concept, it is difficult to define its meaning in bioethics without referring to central theories of justice and equality. Central to this work is the question, where does justice have to focus in order to address existing inequalities in health in order to deliver social justice? Methodologically, the central question of the work is investigated in a case study of dental inequalities in children's oral health promotion policies. The theoretical background of the investigation consists of three theories of justice and equality, namely, the theoretical approach of equality in primary social goods (John Rawls), equality in fairness opportunities (Norman Daniels) and equality of well-being / equality in exercise capabilities (Amartya Sen). Finally, concluding that the demand for justifying principles of justice and equality in population-level bioethics and public health bioethics is strong, the particular oral health debate that has been the case study of our work has led to the proposal of a theoretical justification synthesis. In particular, this synthesis draws from Rawls's undoubtedly influential social justice theory, also from its further elaboration and specialization in health, Daniels' theory but more fully supported by Sen’s theory of exercise the capabilities, a theory that places man in the center of attention, who will achieve, his own well-being by developing his capabilities, contributing, at the same time, to social change and economic development. However, all three theories, despite their differences, can contribute to each other in order to cultivate and develop that theoretical foundation that will ultimately support the demand for the practice of people’s equality. But it remains imperative over time, how the major socioeconomic inequalities, which are the primary causes of inequalities in population health, can be addressed.
